
Assisted Living Facilities
These facilities provide care for individuals who need help with some of
the activities of daily living. Persons requiring assisted living cannot
safely function independently, however they are not in need of daily
medical care.
Continuing Care at Home Program
Recognizing that many older people prefer to continue living in their homes
for as long as possible, the creators of the LifeStyle Solutions Plan have
designed a program that plans for, and delivers, a variety of health,
wellness and home maintenance systems to enable persons to “age in
place” for as long as absolutely possible.
CCRC –
Continuing Care Retirement Community
This retirement option is a community that offers independent
living, outpatient clinic services, assisted living and skilled nursing
all on one campus. The degree to which the services from each of these
areas are included in the monthly fee depends on the contractual
agreement with the community. Some communities, like
Broadmead, are
all-inclusive. This means that most of the campus’ services are included
in the agreement, including all levels of healthcare. Services or
amenities that are often available on a CCRC campus include resident
activities/clubs, meals, housekeeping, maintenance, care coordination,
wellness/fitness facilities and classes, transportation to medical
appointments and shopping, and emergency response systems.
HomeWorks
This is the home maintenance network of providers that is included in
the LifeStyle Solutions Plan. While participation in the LifeStyle
Solutions Plan may make it possible to live at home longer, you
could still be faced with maintenance issues, and with the
need for help with a variety of housekeeping and related activities that
could jeopardize your ability to live at home safely. For all the
small and large challenges associated with living at home, LifeStyle
Solutions offers you HomeWorks, an array of at-home services to enhance
your independence.
LifeStyle Solutions Plan
This is the only program in Maryland – and one of the first in the
nation – to combine several familiar and time-tested concepts into a
comprehensive program that provides you with the services you need to
have a healthy lifestyle, postpone the onset of disability and, should
disability occur, create an effective plan of care aimed at helping you
remain at home, independent and in control. Best of all, the program
comes complete with a network of service providers and the financial
security of long-term care insurance to pay for long-term care services,
if needed. This innovative program is guided in concept and operation by
the caring Quaker philosophy, a philosophy that emphasizes ability over
disability, and the inherent dignity of all individuals.
Long-Term Care
Insurance
This is insurance that helps to cover the expenses associated with
long-term care. Long-term care may be needed for people with cognitive
impairment, disability or a long-term physical illness, and may be
received at home, in an assisted living facility, a nursing home or
retirement community. The financial foundation that supports
the LifeStyle Solutions program is the LifeStyle Solutions Long-Term Care Insurance Plan.
Lead Provider
In Maryland and the District of Columbia, the lead provider for the
LifeStyle Solutions Plan is Friends Care Community Services.
In New Jersey, it is the Community Services Corp. of New Jersey.
Both of these Friends Care, Inc. subsidiaries have the responsibility of
conducting Member Wellness Profiles and developing the provider networks
for Wellness, HomeWorks, and Long-Term Care Services. The lead
providers also work closely with members on member activities that are
coordinated by staff and/or members.
Long-Term Care Management
Because it can be difficult – especially during a crisis – to locate
high quality providers and coordinate their services in a way that's
best for you, the LifeStyle Solutions Plan gives you access to an
integrated system of long-term care providers and highly trained Care
Coordinators to work with you and the providers in the system, cutting
through the confusion and frustration. Should the need for long-term
care arise, your personal Care Coordinator will, with advice and
guidance from you and your family, help create a comprehensive
personalized plan, and act as your personal advocate throughout the care
process. Best of all, long-term care services will be paid for – at the
benefit level you select – through the financial component of the

Nursing Care
Facilities
These facilities, regulated by the state department of health, offer
sub-acute, respite, rehabilitation and other short-term care, as well as
long-term care, to seniors
or anyone in need of nursing care.
Wellness Assessments
Wellness assessments
and profiles are offered to members once each
year. These one-on-one assessments, conducted by our Wellness
Coordinators, look at the complete wellness picture of the member and
provide the basis for wellness counseling that enable the member to
defer disability for as long as possible.
